Abstract: The Scleredema of Buschke is a skin induration
initially affecting neck extended into trunk (chest and
back), face and extremities, with sclerodactyly and multiple
organ manifestations. There is a group of skin
diseases that share the presentation of sclerosis of
the skin, but with different pathophysiological bases
and distinct organic compromises. The simultaneous
associations with multiple myeloma shorten the differential
diagnosis, as usually skin changes may precede
for months or years monoclonal protein detection and
this monoclonal plasma cell dyscrasia is only present
among fibromucinosos disorders (scleromyxedema and
scleredema), having not been described with the rest
of the group scleroderma-like cutaneous syndromes
or scleroderma. The few reports in the international
literature together with the clinical complexity of these
patients highlight the need for multidisciplinary
work, to avoid misdiagnoses and assure them proper
treatment.
