Severe hypercalcemia: unusual presentation of childhood
acute lymphoblastic leukemia (ALL). Case report

Abstract: Acute lymphoblastic leukemia (ALL) presenting
with severe hypercalcemia and lytic bone lesions
is a rare event in children, unlike adults. We report
a 2-years old boy with ALL and hypercalcemia at
presentation. He had normal peripheral white blood
count, with mild anemia, thrombocytopenia, and the
peripheral smear showed type L1 blasts. The bone
marrow examination revealed pre-B ALL phenotype
with aberrant expression of CD13 and normal
karyotype (46,XY) at the cytogenetic study. The
skeletal survey showed scattered osteolytic lesions.
Hypercalcemia was treated with bisphosphonates
with good therapeutic response. He achieved complete
remission, but relapsed early and died with refractory
disease.
