Abstract: The perioperative management of previously anticoagulated patients is a common problem in clinical
practice. The management of these patients is variable and depends heavily on the medical staff involved,
who establishes the risk from thrombotic complications after discontinuation of anticoagulation and the
risk of bleeding before and after the surgical event.
Objective: To describe the clinical characteristics and
the thrombotic and/or hemorrhagic complications of
anticoagulated patients that had surgery or invasive
procedures under general, regional or local anesthesia in the Hospital de San Jose during the months of
September and October of 2009 . The purpose of this
project is the identification of appropriate medical management in the surgical services to reduce morbidity
and mortality related to these complications.
Method: This is an observational study in a prospective cohort of patients who were taken to surgery or
invasive procedures in the Hospital de San José. The
monitoring of the patients was done up to a month
after the procedure performed to establish the thrombotic and hemorrhagic complications after the change
or discontinuation of anticoagulation.
Results: Of the 30 patients, 15 patients had complications; 93.3% (14 patients) were hemorrhagic, and only
one patient had a thrombotic episode. Hemorrhagic
complications occurred in 11 patients who underwent
a major procedure, while only 3 underwent minor
procedures.
Conclusions: In the postoperative period the Hospital de San José documented an increase in bleeding
events compared with thrombotic events. Additionally, it was identified a great diversity in the management
of anticoagulation, with little stratification of the risk
of bleeding or thrombosis, and the usual management
of complications
