Abstract: Prolymphocitic leukemia (PLL) is a rare entity with
its own distinct character. For proper diagnosis
needs to be differentiated from other B lymproliferative
sindromes.
We report the case of a female patient aged 72 with
history of ictiosis and affective bipolar disorder. The
blood test evidenced very high white blood count,
lymphocitosis, anemia and thromobocytopenia. The
fisical examination showed massive splenomegaly
and lymphadenopathy. Prolymphocitic leukemia it
is diagnosed. We highlight
the importance of cytomorphology as the first step
in the differential diagnosis and diagnostic guidance.
The therapy of choice was the combination of
Rituximab, Bendamustine and Mitoxantrone associated
with splenic radiotherapy.
