Abstract: Some acute myeloid leukemias (AML) with genetic
abnormalities have a distinctive morphological
finding. Blast cells have been described with
prominent nuclear invaginations (PNI) called
“cup-like or fishmouth nuclei” associated with
NPM1 and FLT3 mutations. We report the case of
a female patient diagnosed with “de novo” AML,
hyperleukocytosis, cup-like blasts, HLA-DR and
CD34 negative phenotype, normal karyotype and
NPM1 and FLT3-ITD mutations.
