Performance of a point of care device for determination of
international normalised ratio in an anticoagulation clini

Abstract: Vitamin K antagonists are monitored using the International
Normalized Ratio (INR) which is derived from the
prothrombin time.
Over the last years point of care (POC) testing devices
to measure INR have been introduced. The INR-POC
system has been found to be as accurate and reliable as
the traditional method if it was managed by specialized
laboratories.
The aim of this study was to compare the total number
of coagulation visits, patient confortability with a POC
survey and clinical outcome in patients with non-valvular
atrial fibrillation who were anticoagulated with vitamin
k-antagonists. Patients were randomized in two groups:
a) The POC Group with INR determined by the POC testing
device (Coaguchek) and b) Traditional Group with
INR determined by the laboratory traditional method.
One hundred patients were included (50 in each group)
in the study. The survey showed that POC was preferred over the traditional
method. The more important reason for this preference
was the reduced waiting time.
No significant differences were found in the number of
visits to do INR control between the two groups. Patients
who were monitored with POC device presented the same
historical (6 months) behavior. No thrombotic events
during the study in the two groups and only one patient
of POC group presented epistaxis as a minor hemorrhage.
The results of this study showed an improvement in the
patient satisfaction with POC device compared with the
traditional method
