Abstract: Megaloblastic anemia is commonly suspected in the context
of malnutrition and recognized by classical morphological
markers.
The hematologic manifestations of pediatric megaloblastic
anemia, may range from asymptomatic laboratory findings to severe, life-threatening forms.
We report a case in order to illustrate a very unusual shape, but severe megaloblastic anemia presentation in association with pseudo thrombotic microangiopathy (Moschcowitz Syndrome). This entity mimics other serious
blood diseases but not usually present the classic vitamin
B12 deficiency´s hematologic findings which induces the initial suspicion.
The patient received treatment directed to cobalamin deficiency,
without requiring aggressive therapies or costly complementary studies; and shows immediate resolution of symptoms.
Key words: Vitamin B12, megaloblastic anemia,
pseudo thrombotic microangiopathy.
