From monoclonal gammopathy
of undetermined significance
to symptomatic multiple myeloma:
genetic heterogeneity on all levels

Abstract: Multiple myeloma (MM) is preceded by pre-malignant
disease phases of monoclonal gammopathy of undetermined significance (MGUS) and smoldering myeloma (SMM). The genetic abnormalities found in MM comprise of intrachromosomal translocations,
largely involving the IGH locus, copy number abnormalities, somatic mutations and changes in DNA and histone methylation. Many of these genetic
lesions are also present in MGUS and SMM but do not result in the clinical symptoms associated with MM. Here we discuss the common abnormalities
in these disease phases along with the impact of intraclonal heterogeneity on the future of myeloma biology and treatment.
