Abstract: Graft versus host disease (GVHD) is a major complication
of hematopoietic cells transplantation and organ as well as in cases of non-irradiated blood transfusions. Immunological events between the grafted tissue and receiver occur, triggered by their antigenic differences, with clinical manifestations of diverse severity.
We report the case of a 5-year child with diagnosis of LLA B in bone marrow transplantation plan who presents with fever and mucocutaneous commitment one week after receiving a transfusion of non-irradiated blood, is assumed as GVHD-AT, showed good response to treatment with steroids initially, then presents with gastrointestinal and liver disease, rapidly develops multiorgan
failure, unresponsive to supportive treatment and died in the course of one month.
