Recommendations for transient interruption
of anticoagulant therapy in the peri-operative
and invasive procedures

Abstract: Interrupting oral anticoagulation presents a challenge
for physicians; both for those in charge of anticoagulation
and those that should carry out the intervention.
The management in the situations that arise in
these recommendations springs both from literature and
the experiences from authors. The critical point when
suspending the treatment is to avoid the possibilities of
new thrombosis, and at the same time, prevent a bleeding
complication.
To establish the more appropriate conditions in this
prevention/safety balance, some factors to take into account
are proposed: type of surgery, characteristics of the
patient, pharmacokinetic characteristics of oral anticoagulants;
dicoumarin agents (acenocoumarol, warfarin) and
the new antithrombin drugs: dabigatran and anti factor
Xa: rivaroxaban and apixaban, and based on these data,
perioperative suspension schemes are suggested.
