Abstract: Chronic lymphocytic leukemia is the most common type of adult leukemia in the West and is characterized by great variability of presentation and response to treatment. Recently several papers have been published suggesting that vitamin D plays a role in the pathogenesis, progression and even the response to cancer treatment. In order to analyze the values of vitamin D in a cohort of CLL patients and its implications in time to request treatment we analyzed samples of 57 patients. In this cohort values of vitamin D were compared with prognostic factors available for most of the centers in our area (LDH, β2-microglobulin, ZAP-70, CD38) and Binet stage at diagnosis. A relationship between Vitamin D deficient values and increased LDH and β2-microglobulin (p = 0.022 and p = 0.08) as well as a shorter time of treatment was demonstrated. The hazzard ratio (HR = 2.606) allowed us to infer that patients with this deficiency are 2.6 times more likely to initiate treatment. So far the value of vitamin D seems to be the only modifiable prognostic factor, so this finding could change the natural history of the disease.
