Abstract: Splenic peliosis is a rare entity, yet of unknown etiology: toxic, infectious related to inmunodeficiency, inflamatory of vascular. It is characterized by multilocular blood filled spaces, that would led to hemostatic dyscrasias or extreme splenic fragility with easy or spontaneous rupture, often without enlargement of the spleen. Even, extremly uncommon is its association with Chronic Myelomonocityc Leukemia (CMML), a diagnosis controverted chronic myeloproliferative syndrome. We have studied in detail the bone marrow, the spleen and the cytogenetics and we concluded that CMML is a true myeloproliferation. We also suggest a strict clinical, hematologic and radioimage surveillance for an appropiate evaluation of those diseases which course with huge spleens at any time of its evaluation.
