Abstract: Heparin-induced thrombocytopenia type II as an immune origin and is characterized by a decrease in platelet levels, generally after the fifth day of therapy and a severe and paradoxical acquired thrombophilic state. Only heparin withdrawal leads to the resolution of the disease. A high index of suspicion is necessary, because the clinical manifestation can be masked by the underlying entity for which heparin was indicated. The aim of this article is to review the clinical manifestations and the state of the art of the confirmatory lab test. International guidelines for the therapeutic management of HIT can not be followed in Argentina due to the lack of the three most useful drugs in this setting: hirudin, danaproid or argatroban. Therefore this dangerous situation should be a warning to the Health autorities as well as to the Pharmaceutical enterprises of our country.
