Abstract: Minimal rsidual disease (MRD) allows detection of blasts in patients with morphologically complete remission. In patients with ALL (Acute Lymphoblastic Leukemia), is an independent risk factor associated with lower event free survival (EFS). From January 2006 to Decembre 2009, 84 pediatric patients with newly diagnosed ALL were treated according to ALLIC-BFM-GATLA 2002 protocol. The cutoff used to define MRD positive in ALL was 0.01%. Positive MRD at day 15 were found in 57.5% of patients and was correlated with higher leukocite count al diagnosis, high risk, relapse and lower EFS and OS (Overall Survival). Positive MRD al day 33 was found in 13.3% of patients and was significantly correlated with higher blood cell count al diagnosis, T phenotype, poor response al day 8, high risk, relapse and lower EFS and OS. MRD at day 15 and 33 of treatment is an important prognostic factors to predict relapse in pediatric ALL.
